Full Job Description

  • Develop and sustain productive working relationships with colleagues to provide clinical support to Clinical Advisors and Call Centre Managers on shift, ensuring safe effective working.

  • Review patients awaiting contact to provide oversight and minimise clinical risk by actively filtering for potential signposting to more appropriate services.

  • Based around a Red Flag approach of triage, prioritise patients in line with the service escalation plans which include the elderly, palliative patients and those who have a risk of deterioration.

  • Apply clinical skills and reasoning to ensure patients and transferred safely between different clinical dispositions and signposting when required and safe to do so.

  • Lead shift safety huddles to provide updates and information to clinical teams. Acting as single point of contact and oversight for referrals and co-ordination of patients awaiting contact

  • Liaise with nominated medical/nursing leadership team to provide oversight and co-ordination of patients awaiting contact.

  • Undertake robust clinical triage and allocate to appropriate clinical teams based on available skill mix.

  • Identify high risk groups or patients who may need escalation based on their clinical priority.
