Full Job Description

To provide care and support to vulnerable adults who require high levels of support.

Provide person-centred care and support to individuals to ensure they live life to the full.

To promote independence and maintain equality and diversity at all times.

Notes for reference.. Any staff which hold a NVQ level 3 regardless of their position may be required to act as the responsible person if a more senior staff member is absent or in cases of emergency, this is up to the discretion of the manager.

General Duties

1. Update "Care plans" promoting a person centred approach.

2. To complete "Medication Training" and administer when training has been completed, to complete appropriate records.

3. Deliver all aspects of personal care.

4. Bed making and ensure all residents bedrooms are kept in clean and orderly manner at all times.

5. Observing and recording of fluid intake orally and urinary output.

6. Record the weighing of residents as per care plan request.

7. To support the resident to medical or other professional appointments.

8. Communicate information verbally and / or in writing effectively.

9. Communicate information to different professionals on request.

10. Establish excellent relationships with our residents/customers and their families.

11. Assess and / or anticipate customer needs. (Record in their care plan)

12. Assess service delivery requirements.

13. Exercise customer focus skills.

14. Planning and organising within the workforce.

15. Deal with and solve problems in line with legislation.

16. Analyse and interpret information to professionals when requested...

17. Demonstrate own duties to other employees.

18. Ensure accuracy in work at all times.

19. Ensure work is carried out in an organised and effective way.

20. Maintain focus on tasks, even in challenging circumstances.

21. To ensure their training is up to date, ensure mandatory training is completed as well as ensure there is an update DBS certificate in place.

22. Deal with deadlines.

23. Deal with interruptions and conflicting demands.

24. Work effectively with others to build relationships and find common solutions.

25. Contribute to the effectiveness of improvement activities within the home.

26. To arrange and support and take part in social, recreational and spiritual activities provided for the resident..

27. Use creative skills to develop novel or unique ideas.

28. Implement and / or co- ordinate new technology and processes.

29. To work on a 24hr 7 day a week rota which can be changed given 4wks notice and up to the discretion of the manager

30. All staff to be made champions of subjects which will be up to the discretion of the manager.

31. Accountable to the "Care coordinator"/manager

32. To maintain strict confidentiality at all times

  • All of the above to be in consultation with the manager.
