Full Job Description

Job resonsibilities Patient care You will be responsible for a group of patients without direct supervision. As a registered, accountable, and autonomous nurse to always act in the best interests of your patients and take personal responsibility for your actions and decisions. To be responsible for patient assessment, planning of care, giving, and supervising safe and compassionate care, and evaluating the effectiveness of care. The post holder will be responsible for the assessment of care needs and the development of programmes of care ensuring that these are evaluated and amended as appropriate. Promote the health and well-being of your patients, clients, and groups. Undertake and document a comprehensive, systematic, and accurate nursing assessment of the physical, psychological, social, and spiritual needs of your patients, clients and their carers. Formulate and document a plan of nursing care, where possible in partnership with your patients, clients, their carers, and family and
friends, within a framework of informed consent You will ensure patients dignity and privacy is maintained at all times. Monitoring and assessing patients condition, responding to changes in condition, proposing/making changes to care as required. Recognise if your patients health is deteriorating, manage the patient, escalate your concerns and take appropriate action. Evaluate and document the outcomes of nursing and other interventions. Demonstrate sound clinical judgement across a range of differing professional and care delivery contexts Handover and transferring responsibility of care to colleagues, ensuring appropriate information regarding patient care requirements is relayed. Reporting and documenting all relevant patient needs, communicating changes to other health professionals in the multidisciplinary team.
