Full Job Description

The key objectives for this post are to provide goal focused rehabilitation, supporting patients to become experts in their own care and to remain in their own homes by providing holistic assessments and interventions for this care group. You will be supporting the Discharge to Assess pathway and the Urgent Community Response as part of this.

As a Specialist Physiotherapist, you will have the opportunity to undertake a leadership role in planning, coordinating, delivering, and evaluating the rehabilitation provided and be responsible for the supervision of unregistered staff and students.

You will be able to demonstrate the ability to build networks and relationships over the wider integrated neighbourhood team and MDT. You will embrace innovation and integrated working and will be committed to delivering excellence in care alongside other Physiotherapists, Occupational Therapists and Assistant Practitioners.

We are looking for an experienced, motivated clinician to join our team. You will be a registered Physiotherapist with demonstrable expertise in goal focused rehabilitation to provide high quality therapy interventions for the patients within the South Rural INT.

It is expected that you will have excellent communication skills, enjoy working with a variety of service users of different age groups, have good self-management skills and an ability to prioritise your case load. The South rural Integrated Neighbourhood Team is based at Beacon House, 53 Whitehouse Road, Ipswich IP1 5PB.

The service covers the core hours of 08.00-18.00 over 7days and you will be expected to work flexibly within these hours.

The post is full time of 37.5 hours, but we would consider a part time worker.

It is essential that you are a car driver, with access to a vehicle for work as you will be expected to travel across the South rural area.
