Full Job Description

To support and assist in the provision, development and delivery of a responsive, proactive and patient-focused pharmacy service to the ward based clinical teams.,

  • To interpret and dispense inpatient, outpatient and discharge prescriptions in accordance with departmental procedures. This will include prescriptions for oral cytotoxic and controlled drugs.

  • To place completed inpatient orders into patient lockers.

  • To assist the ward pharmacists and technicians in discharge planning at ward level and to work with Staff Nurses and other healthcare professionals to facilitate timely discharges.

  • To dispense ward discharge prescriptions and deliver them to the ward areas to facilitate timely discharges.

  • To prompt the ordering of medication by liaising with ward staff and by using the Electronic Prescribing and Medicines Administration system.

  • To check patient POD lockers when appropriate, removing stopped medication and communicating with patients when necessary to ascertain supply requirements. To prompt relabelling of items when appropriate.

  • To ensure seamless, accurate communication with pharmacy and ward staff regarding medication issues, particularly with regard to urgent supplies and discharges.

  • To liaise with Provision and Procurement in relation to stock replenishment. To communicate stock issues to the ward teams and ward staff where necessary and to help manage the Ward drug trollies.

  • To refer queries regarding the storage of refrigerated items to the Medicines Information department or appropriate pharmacist.
