Physical Healthcare Practitioner

We are looking to recruit an experienced to an exciting Physical Healthcare Practitioner (PHCP) part-time ( hours) position - Inpatient Mental Health Services Older People (MHSOP) in Scarborough and Malton.

We are seeking a motivated, creative team player with demonstrable leadership experience to help us build the skills, competencies & confidence of our staff across MHSOP wards in the diverse localities of Scarborough & Malton.

This is a perfect opportunity for an existing Senior RGN working within Acute or Primary Care Services looking to diversify their professional portfolio or someone ready for a new challenge & development opportunity.

You will be a valuable addition to our highly-skilled multidisciplinary teams, working flexibly alongside another part-time PHCP, to prioritise your presence & input across 2 wards, dependent on inpatient needs.

You will work at :

• Rowan Lea, Cross Lane Hospital (Scarborough) : 20 bed assessment & treatment unit for men & women (range of mental health illnesses)


• Springwood (Malton) : 14 bed unit for men & women (dementia)


We welcome applicants who have significant experience of delivering physical health interventions to older adults with a range of complex needs, including organic or functional mental illness, falls, frailty, polypharmacy.

Main duties of the job

As a Physical Healthcare Practitioner , you will have a key role in supporting MHSOP inpatient teams by working as part of the Multi-Disciplinary Team to develop and implement treatment and care plans and to support service improvement within this area of practice.

You will work closely with the Physical Health Modern Matron and clinical teams to provide a high quality service to patients through support, intervention and clinical expertise on the various aspects of physical health including : assessment, diagnosis and review, chronic disease management, management of acute presentations, palliative and end of life care and ongoing physical health monitoring.

The role also involves collaborative working with the Trustwide Lead for Physical Health on various physical health work-streams, and the delivery of staff training, education and development.

You will support evidence based practice and have experience of working independently & autonomously in a Nurse Practitioner role, utilising clinical assessment skills and demonstrating sound clinical decision making.

You will have oversight of a patient caseload, whilst supporting the strategic role of developing physical healthcare across the Trust., Please see the attached job description and person specification, which outlines the main duties and responsibilities of the role.


• First level Registered Nurse Adult (RNA) or Registered General Nurse (RGN) with current NMC registration and eligible to practice, or Registered Allied Health professional (AHP) with current HCPC registration and eligible to practice


• BSc Nursing (or AHP equivalent), or post-registration specialist course at BSc level


• Post-registration teaching and assessing qualification ( Mentorship, ENB 998 or City and Guilds 730)


• Post-registration Physical Assessment Skills (Adult) Level 6 qualification


• Independent prescribing Level 6 qualification (or willing to work towards within 12 months)


• Evidence of continual professional learning and development


Desirable

• Registered Mental Health Nurse (RMN) or Registered Nurse Learning Disabilities (RNLD) with current NMC registration and eligible to practice


• MSc Nursing (or AHP equivalent) or post-registration specialist course at MSc level


• Post-registration Advanced Physical Assessment Skills and Disease Management Level 7 qualification (or equivalent)


• Advanced Pathophysiology Level 7 qualification (or equivalent)


• Clinical Assessment of Patients with Long-term Health Conditions Level 6 qualification (or equivalent)


Experience

Essential

• Extensive clinical knowledge and experience


• Substantial experience of working clinically at Band 6 or above


• Ability to assess and implement a range of evidence based therapeutic care interventions


• Proficient in multi-agency partnership working

• Demonstrable knowledge at post-graduate level of physical healthcare


• Advanced knowledge and understanding of physical examination and assessment


• Substantial knowledge of working at Band 6 level or above utilising clinical assessment skills


• Knowledge of national policy and guidance in relation to aspects of physical health


• Knowledge and experience of clinical audit and research


• An understanding of clinical governance and the application of the clinical governance agenda in the NHS


• Knowledge of incident reporting and the importance of incident investigation


Desirable

• Previous knowledge of mental health care or working within a mental health or learning disability environment


• Knowledge of mental health problems ( specific mental illness, personality disorder, eating disorders etc.).


• Evidence of further education in research or audit

• Advanced skills in physical examination and assessment


• Appropriate technical skills ( venepuncture, ECG monitoring etc.)


• High level of record keeping and documentation


• High level of interpersonal and communication skills


• Ability to plan, set objectives, priorities and review performance


• Computer and keyboard skills and use of Microsoft packages
