Full Job Description

Gateshead Inner West PCN are seeking an experienced,self-motivated, hard-working, and patient focused nursing associate to joinour team.

The post holder will work closely with our GP's and other primary careprofessionals within the PCN to proactively identify and manage a caseload ofpatients within the community, making sure that support and care is made available to them andthat their complex needs are addressed.

This role is offered on a full-time basis, for hours per week, Monday - Friday.

Main duties of the job

  • To support the organisation and facilitation of clinical care, in a holistic and proactive manner, to patients registered within our PCN.

  • To maintain a high professional standard of nursing care, in adherence to all PCN protocols, policies and guidelines, as well as to provide nursing assistance to the GP's and other members of the primary healthcare team

  • To demonstrate flexibility, in line with the present atmosphere for change within the health service delivery.

  • To work within your own level of competency, and expanding the role in accordance with PCN requirements and own scope of practice, Deliver high quality compassionate care

  • Undertake clinical tasks including, venepuncture, ECGs, general assessments, NHS health checks, wound care and management.

  • Health promotion - alcohol screening, smoking cessation, healthy diet, physical activity, weight management, sexual health, and family planning.

  • Long term conditions screening, on going management, and referral to other agencies

  • Excellent verbal and written communication skills

  • Abilityto work as part of a team.

  • Excellentorganisation and time management

  • Handle sensitive and confidential information.

  • Ensuring the privacy, dignity and safety of individuals is always maintained.

  • Recognise opportunities to reduce inequalities and unwarranted variation

    Registered Nursing Associate on the NMC register.

  • Nursing Associate foundation degree qualification

  • GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent

  • Hold Full UK Driving licence and access to a vehicle insured for work purposes is essential for this position


  • Experience

    Essential
  • Experience of working in a multi-disciplinary team

  • Experience of dealing with vulnerable and complex patients

  • Experience of providing and receiving complex, sensitive information

  • Experience managing long term conditions such as Asthma, COPD, Diabetes, CHD


  • Desirable
  • Experience and knowledge of Primary Care Networks and / or General Practice

  • Experience in using EMIS

  • Experience in Immunisations (National programmes), Cervical Cytology

    Based in the InnerWest of Gateshead our Primary Care Network serves over 45,000 patients acrossour five member Practices of : Whickham Medical Cottage, Chainbridge Medical Partnership,Glenpark Medical Practice, Teams Medical Practice & Sunniside Surgery.
