Could your skills and experience help vulnerable people who access health care more than expected?,

  • Experience in delivering casework within areas of vulnerability ; mental health, drugs and alcohol abuse, physical health conditions.

  • Experience supporting people with complex needs. You'll be authentic, relatable and caring. You'll know how to anticipate, identify and resolve risks and issues.

  • Experience in person centred support planning and dynamic risk assessment.

  • Strong negotiation skills and you can think on your feet.

  • Meticulous record keeping skills.


  • Within this role there may be a need to visit remote locations which have no access by public transport. Therefore you must have a UK driving licence and access to a vehicle or be eligible for transportation via the Access to Work Scheme.

    Are you an experienced Outreach Caseworker in Mid Essex area? Would you love to work on an innovative NHS programme that helps support people who access health care more than expected (High Intensity Use (HIU) Programme)? The aim is to change the perceptions for this group of vulnerable people. The team help to prevent people from reaching crisis point, empowering them to better self-manage and connect with their surroundings.
    What does being an Outreach Caseworker involve?
  • Working in the community to help identify and address unmet social needs that may be exacerbating physical and/or mental health. Working to identify what causes high usage of emergency care services.

  • Helping people with complex presentations including mental health, substance abuse, multiple long-term conditions and adverse traumatic experiences. They're often suspicious of formality and offers of support. Your experience and lead will help them access the appropriate help.

  • Helping to lower stigma within the community and change mindsets and behaviours. You'll achieve this by building strong relationships with partners and stakeholders, such as A&E departments, Community Mental Health Teams, GP surgeries, housing teams etc., In return for your dedication and expertise, what will you get?
