Full Job Description

As a Mental Health Care Co-ordinator, you will be responsible forsupporting the practice in delivering excellent patient care by effectivelyco-ordinating different elements of care from multi-disciplinary teams withinand outside the practice.

We are currently seeking to recruit a new mental health focused CareCo-ordinator to join our expanding Proactive Care Team.

Working alongside our existing CareCoordinator team, Social Prescribing Link Workers and Health & WellbeingCoaches as well as our GPs and clinical teams, you will triage patients and coordinatecare with a mental health focus.

Main duties of the job

Our successful candidate will have a busy and varied role focussingon mental health including service co-ordination, care planning for patientsand providing direct patient support.

You will co-ordinate multidisciplinary team meetings, liaise withclinical and non-clinical teams within Holderness Health and across our partneragencies.

Acting as a key point of contact for patients, carers andhealthcare professionals, you will ensure that an individuals healthcare needsare addressed in a joined-up way, ensuring that they receive the right carefrom the right professional at the right time.

You will have excellent communication andnegotiation skills and be confident managing your own workload. Our ideal candidate will have experience ofworking in a health or social care setting and a deep understanding of patient careneeds but we are also interested in hearing from people from a wide range ofbackgrounds who can demonstrate sound administrative skills and experience ofdelivering high quality customer or patient care., To support the practice in delivering excellentpatient care by effectively co-ordinating different elements of care frommulti-disciplinary teams within and outside the practice focussed on mentalhealth

ServiceCo-ordination

Provide coordination and navigation for patientsand their carers across mental health and care services, working closely with SocialPrescribing Link Workers, Health and Wellbeing Coaches, and other primary careprofessionals

Co-ordinate the work of healthcare professionalsand non-clinical staff involved in the care of patients registered at thepractice

Co-ordinate MDT meetings, collating informationon patients requiring review and providing secretarial and administrativesupport, and ensuring the completion of resulting actions

Act on communications from hospitals, communityservices, mental health services and other providers

Facilitate inter-agency communication to supportthe discharge and handover of patients between different mental health and caresettings

Support Lead GPs with the administrative aspectsof QOF (Quality Outcomes Framework)

Act as a named, single point of contact forhealth professionals and patients within your remit (mental health focus)

Point of contact for patient mental healthqueries and triage / sign posting into appropriate mental health services

Assist with the marketing of the Proactive CareTeam through event planning and attendance, newsletters and social media.

Maintenance of the practice Health Corners toensure up to date and relevant information exists.

Assisting the Social Prescribing Teams withreferrals

Patient Support& Care Planning

Contact patients with mental health concerns andqueries to assess, triage and signpost into appropriate mental health services,seeking clinical guidance where required

Make every contact count - help people to managetheir health needs through answering queries, making appointments, bookingtests and arranging other reviews including long-term condition reviews,screening and vaccinations.

Ensuring that actions from review meetings areprogressed on behalf of patients

Provide patients with good quality written orverbal information to support them in making choices about their care

Proactively support patients to participate inlocal and national screening programmes

Support patients in readiness for shareddecision-making conversations

Support people to understand their level ofknowledge, skills and confidence engaging with their health and wellbeing

Identify and address safeguarding concerns

Identify and support carers of patients toensure that they also look after their own wellbeing

Work with GPs and other primary careprofessionals to identify and manage a caseload of patients

Supporting patients through the varying stagesof mental health such as : -

Prevention and stayingwell

Provide single point ofcontact to be able to assess and respond to changing needs

Provide education around self-care,self-management and health literacy

Support people to get the mostout of consultations, reducing the need for follow ups

Facilitating joint working andinformation flow across mental health teams and health care systems.

Reducing the risk ofdeterioration through personalised care and regular contact.

Deterioration / acuteepisodes

Facilitate referrals andensure timely access to right professionals and teams

Support with coordination oftreatment escalation plans

Help identify where peoplesneeds are increasing, and they are at risk of an acute episode

Discharge and recovery

Provide support to people whohave been discharged to ensure all necessary arrangements in place

Support people to access theright service and resources for them

Empower and enable people toself-manage

Facilitate joint working andinformation flow across teams and healthcare systems.

Actively promote your role within the practiceand PCN

Raise awareness of Proactive Care Team in thecommunity by attending events and community groups

Contribute to tackling inequalities in healthand social care particularly regarding individuals with long-term conditionsand complex care needs

Support the development and implementation ofprojects, campaigns, education and initiatives aimed at improving mental healthand care of patients

Ensure personal skills are kept up to date bycomplete mandatory training and additional training

Reporting

Maintain effective records of activity andproduce reports as required

Ensure personal workplan is up to date

Provide any agreed performance / activity data

Assisting to document and evidence outcomes

Qualifications

Essential

Good standard of general education (NVQ Level 2 or equivalent)

  • Experience of one or more of the following areas : mental health, community services, palliative care, learning difficulties, dementia, care homes, mental health, long-term health conditions, frailty, substance misuse
