Macmillan Cancer Support and the Cheshire & Merseyside Clinical Network, in collaboration with The End of Life Partnership (EOLP), are looking for a Palliative and End of Life Care (PEoLC) Transformation Lead to join our team. This new post brings an exciting opportunity to play a leading role in the strategic planning, delivery, and sustainability of palliative and end of life care across Cheshire & Merseyside.

As the Macmillan Transformation Lead, the post holder, whilst employed by EOLP, will work alongside the Cheshire & Merseyside ICBs PEoLC Programme team to improve outcomes for patients by improving access and sustainability to PEoLC services and reducing inequalities. You will work closely with partners, commissioners, and providers to promote best practice at a system level. Working across geographic and professional boundaries, across multiple care settings and communities transforming end of life experience and care across all ages, backgrounds, and conditions., The post holder will Programme Manage the C&M ICS PEoLC Transformation Programme. The role will also involve;

  • Increasing system level profile of PEoLC through ICS engagement and the development of the strategy for C&M

  • Improving the quality of data reporting evidenced through compliance with Key Performance Indicators

  • Improving sharing of PEoLC information across the system evidenced through the PEoLC data dashboard.

  • Understanding variation of access to services and in delivery of care evidenced through AMBITIONS self-assessment across 9 PLACE based partnership areas.

  • Improving collaboration with those who have lived experience by growing the membership of the Peoples Voice group representing diversity of the ICS.

  • Creating and agreeing a strategy for PEoLC for C&M in collaboration with the ICB Population Health clinical executive lead agreed through the ICB and implementing through PLACE based partnership leads.


  • Whilst EOLPs main office base is near Crewe in Cheshire, the post holder will be required to travel and work across Cheshire & Merseyside Integrated Care System.
