Adferiad provides a 24 hour temporary Supported Accommodation to individuals with a serious mental illness in Llanelli, Carmarthenshire. The Llanelli service is delivered within a shared accommodation for six adults or in peoples own homes in the locality. Our clients have experience of serious mental illness and need a medium level of support to help with their recovery.

The service is staffed 7 days a week, and is offered to clients on a short to medium term basis according to needs.

The aim of the Service Manager and staff team is to encourage and enable clients to reach a greater level of independence and then to facilitate their move on to more independent living in the community, supporting the wellbeing initiatives as identified within Welsh Government policies and the Social Services and Well-being Act (Wales) 2014.

The role of the Service Manager is to ensure the timely, efficient and effective management and delivery of services within the project in line with our commitment to the principles of empowerment, self-management and recovery and ensuring compliance with internal and external quality standards inspection and regulation (including CIW).

Main duties of the job

To work with Commissioners, Community Mental Health Teams and other partners to manage & deliver a high specification service and regulations.

The Service Manager will take the lead on processing of approved referrals, following procedures for completing new assessments, risk assessments and creating individual person-centred support plans and keeping Carmarthenshire County Council Commissioners & relevant teams informed of decisions and progress.

Follow procedures for conducting 3 monthly support plans reviews or as and when required due to changes in individuals needs or circumstances, ensuring risk assessments, support delivery plans are kept up to date and are relevant.

Work with individuals to achieve their outcomes and the outcomes of the project, using a person centred approach to assessment and support planning employing creative and innovative methods to meet these outcomes and, through the Managers & staffs experience and resources, bring added value.

Ensuring support is provided based on an individuals needs, desires, and personal preferences to promote wellbeing and independence whilst adhering to the services Contract and all relevant policies and procedures internal and external.
