As a Community Transition Coordinator within our Brimscombe based Hub Academy team, you'll play a pivotal role in facilitating the smooth transition of residential treatment clients back into the community. Your responsibilities will include leading a program to enhance independence, fostering progression routes, and developing robust move-on plans for sustained recovery post-treatment.
