Full Job Description

Initial supernumery period and mentored role to develop accountability for his/her own professional actions. Supervised assessment of undifferentiated patients with a range of acute, non-acute and chronic medical conditions. Develop core knowledge and competence to a take history, make appropriate physical examinations, formulate differential diagnoses and formulate/manage treatment plans when working in any clinical environment. Refer and facilitate patient pathways through appropriate referrals to primary, community, secondary and tertiary care for patients when necessary, such as 2-week wait, urgent and routine referrals to specialities. Develop the skills and knowledge to recognise the need and initiate emergency treatment to save life or to safeguard the interests of the patient and refer to emergency services when appropriate. Develop the skills and knowledge to assess and manage a wide range of minor injury and illness presentations Develop competence to recognise any social
needs, safeguarding concerns and act appropriately to safeguard the patient. Develop competence to ensure that patients are safely discharged from our care with appropriate instructions for after care and follow up. Identify opportunities to proactively encourage health promotion and health education to all patients. Take a person-centred approach to service delivery, based on clear understanding and experience of the modes for service delivery. To assist in the development of new models for service delivery which have a positive impact on the health economy. Develop and maintain clinical competence and knowledge of care delivery within the health care setting taking responsibility for your educational and developmental requirements and continued professional development. To work within the professional Code of Conduct and other directives as defined by the NMC/HCPC. To be responsible and accountable for legible and contemporaneous clinical records To undertake appropriate personal and
professional development to meet the requirements of NMC/HCPC Registration. To participate in regular performance review with your clinical line manager.
