Full Job Description

The role of the Care Co-Ordinator includes all aspects included within the Support workers role.

The role is to undertake those duties required of them as delegated by the Manager or Deputy Manager. The delivery of care will be determined by the individualised care plans and the Care Co-ordinator will report regularly to the Manager/Deputy Manager, passing on any changes that are observed or problems identified.

Under the supervision of the Manager/Deputy Manager, care for all the residents in the Home in a sensitive, tactful and compassionate manner, bearing in mind the need to provide a happy, homely atmosphere.

The Care Co-ordinator will be required to work the staff rota covering evening work although may be required to cover day shift.

The Care coordinator will be respected as part of our management team and will maintain a professional role working for MCHG.

General Duties

1. To compile a three week in advance rota for the staff and ensure it is available to all staff. This is up to the discretion of the manager as some managers will complete the rota.

2. To maintain records of annual leave and ensure that staff take all leave in appropriate manner.

3. To carry out staff supervisions as required but at least every 8 weeks. (6 per year)

4. To assist in all quality assurance procedures

5. To work with the Deputy manager to complete risk assessmenys where applicable.

6. To assist with all duties included in the role of the Support worker.

7. To aid in the training and development of other Care staff and up date records accordingly.

7 To act as a role model of good practice for other care staff to follow.

8 To demonstrate commitment to the development of the company through interactions with staff, residents and members of the general public.

9. To ensure that staff follow policies and procedures at all times and to ensure staff have signed all related documents.

10. To arrange and take part in social, recreational and spiritual activities provided for the residents.

11. To maintain the strict confidentiality which surrounds the residents and the business of the Home generally.

12 To ensure the safe ordering, administration and recording of prescribed drugs to appropriate residents.

13. Assist the Deputy Manager with diciplinary investigations where appropriate.

14. To work with the Manager to structure their workload in order to allow for office based duties.

15 Support the Deputy Manager to chair staff meetings on a monthly basis.

A job description is not a rigid document but acts to provide guidelines to the duties expected of the post holder. Other duties may be required at the discretion of the Manager.
