Full Job Description
Our Reception/Admin team are the first contact patients have with our services. The role of the care co-ordinator is to make sure we deliver the best possible care and experience for our patients. Successful candidates will need to be comfortable with a flexible approach to tasks and problem solving. You will work alongside the admin team carrying out duties for the Practice Team relating to patient care.,
- Dealing with patient queries, tasks and delivering messages from both the Clinical and Admin team.
- Prepare information and test requests for GP's and patients
- Manage Patient waiting lists and appointments
- To help and assist with co-ordination of patient contact for annual chronic disease reviews to help hit QOF targets
- Assist the admin team with the planning and scheduling of annual recalls within the QOF year.
- Work as part of a team to carry out searches and audits to ensure patients' needs and records are kept up to date and accurate.
- Use information gained from searches and audits to arrange appointment, immunisation recalls and health checks in line with locally commissioned services
- Handle Practice Registrations and deductions
- Manage electronic GP2GP links and resolve degraded information
- Provide additional administrative support to the workflow redirection team processing incoming documentation in a timely manner.
- Encourage the uptake of our digital tools to support patient care
- To summarise incoming medical records for new patients and code any relevant information
- Manage recalls - to send out reminders to patients to come in for a follow-up appointment.
Work effectively with colleagues within the admin team - A willingness to learn and progress with a can do attitude
- System One knowledge and experience is desirable
- Experience of data entry and coding
- Experience in communicating with members of the public confidently.
