An exciting and rare opportunity has arisen for an experienced Children and Young Peoples Nurse to join the Post Covid multidisciplinary Team. The role will contribute in the delivery of high standards of health care to patients with post covid syndrome both remotely and in face to face clinics across the area of Norfolk and Waveney.

The team are constantly striving to improve care for children with Post-COVID syndrome. The role requires engagement with other healthcare professionals to support the modernisation of care delivery with frequent supervision.

We are looking for the right people for our team, who have a passion for patient centered, supportive care within a multi-professional team, with prior expertise, knowledge and training in complex, patient focused assessment, goal identification, coaching and self-management with a strong understanding of physical health and mental wellbeing and the possible symptoms and complications for patients in Post-Covid syndrome.

  • Support Team Lead in the development and maintenance of policies and procedures

  • Lead where required on the development and maintenance of processes and pathways

    Job Purpose
  • To work as an autonomous practitioner leading within the Post covid service to provide clinical intervention and support to children, young people and their families who have complex or acute health needs as a result of post covid syndrome

  • To be a senior clinical resource to work with the Team Lead on the implementation and development of the Post-COVID Service.

  • To identify future opportunities to drive subsequent phases of the Post-COVID Service in line with NHS England National Strategy.

  • To be involved and lead when required in the delivery of high standards of health care to any patient registered with a Norfolk or Waveney GP, by coordinating and delivering assessments, planning and implementing and evaluating health care for patients following COVID.

  • To co-ordinate practice, procedures / systems for the team and provide clinical support within own area of expertise including clinical assessment delivery.

  • To develop and sustain working relationships with the partners delivering the Post-COVID Service processes and patient focused planning.


  • Key Areas of Responsibility
  • Clinically lead on service development, performance, management, and profession of the Post-COVID Service related to children and young people, and to further develop the service as required.

  • To participate on the development and maintenance of policies and procedures for Post-COVID Service related to children and young people,

  • To participate in the development and maintenance of processes and pathways for Post-COVID Service related to children and young people.

  • To provide professional leadership to own professional group and Post-COVID Service colleagues where appropriate, and develop the role of Post-COVID Service particularly related to children and young people

  • To work with all health, social care, and wellbeing professionals, and statutory/non statutory agencies to provide a holistic and seamless, integrated, service to our patients

  • To identify and lead service development opportunities as required within Post-COVID Service


  • Clinical
  • Support Team Lead in the development and maintenance of policies and procedures within the Post-COVID Service related to children and young people.

  • Lead where required on the development and maintenance of processes and pathways for Post-COVID Service particularly related to children and young people.

  • To review referrals, support triage and liaison for patients referred into Post-COVID Service particularly related to children and young people,

  • To provide quality care to children in partnership with families and carers as commissioned and described within the service specifications.

  • Supporting the MDT process and lead where required at Post-COVID Service, review patients' holistic needs, communicating highly complex and sensitive information to referrers, and colleagues, patient and carers as required.

  • Where there are barriers to understanding, such as cultural barriers, hearing impairment, mental capacity impairment and other difficulties in comprehension or language impairments, awareness of reasonable adjustments.

  • Use risk stratification methods in partnership with other professionals to identify those patients most likely at risk of health, social care and wellbeing crisis and focus support to ensure the patients independence is maximised.

  • Identification and awareness of inequalities in health outcomes and access to support for Norfolk and Waveney citizens.

  • To take all possible steps to ensure the safety and welfare of children and staff

  • To offer advice, guidance and reassurance to families as needed

  • Be required to use tact and persuasive skills in order to coordinate and plan on-going care, treatment, or management with colleagues, for patients.

  • Liaise with GP's, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.

  • To participate and lead (where appropriate) in multi-disciplinary / multi-agency meetings as appropriate.

  • Provide clinical expertise and advice to others, including social services, GP's and other agencies.

  • Ensure activity data and clinical information is recorded on system one and other tools as required, to be completed on a daily basis.

  • Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advances sensory skills, manual and mobilising skills.

  • Provide initial triage and assessment for new referrals to the service, delegating to appropriate others as required and incorporating secondary or onward care referrals as required.


  • Professional

    The post holder will:
  • Be required to undertake risk assessments and act upon them appropriately

  • Be responsible for ensuring workload for self and others in planned and prioritised according to service targets and needs

  • Ensure own paperwork and records are maintained, and those maintained by the team

  • Contribute to the completion of the staff rota on a monthly basis

  • To initiate and contribute to the development and introduction of new ideas and ways of working and act as a source of advice to the benefit of the child and your person and the development of the service

  • Contribute to the supervision of the workload demand and resource utilisation within the team

  • Order equipment and supplies via the electronic ordering systems used within the team

  • Actively contribute to the multidisciplinary team meetings and other meetings where appropriate, and chair meeting where required

  • Develop and maintain effective working relationships and communication with all members of the organisation, other professionals, and agencies

  • Instigate, and actively participate in the development of the operational strategy and project management

  • Ensure effective use of resources, both financial and human, by members of the team

  • Participate in the recruitment process, using the opportunity to skill mix to meet the demands of the service

  • Attend in-house training as required to develop current job role

  • Will be able to make own travel arrangements as required
